The Affordable Care Act introduced two Alternative Payment Models – bundled payments and Accountable Care Organizations. It also introduced the first ever Hospital Readmission Penalty, Medicare Spending Per Beneficiary Penalty and other penalty program to speed up the transition to value based care. Specific attention is given to the new, 2019 CMS discharge planning rules that require hospitals to meet certain communication, medical record and transfer requirements. Having penned the best selling book of 2015 for the American College of Healthcare Executives titled Readmission Prevention: Solutions Across the Provider Continuum, former hospital CEO and nursing home administrator Dr. Josh Luke is the foremost hospital and post acute authority on the transition to value based care. Handouts & Tools

This presentation includes two handouts to be used as resources for attendees. First, the copyrighted Discharge with Dignity Guide that has been used by multiple health systems nationwide to train doctors and discharge panning staff on how to safely discharge patients directly home to avoid  the nursing home when safe and appropriate. The second tool is t eh Emergency Department Alternative Discharge Guide that led to the phrase “Why not home?” and teaches emergency physicians and discharge planners the other options that the Medicare benefits covers for direct transfer of patients from the emergency department. 100 complementary “Why not Home?” buttons are included with this presentation for attendees and are very popular.

About the Speaker, Dr. Josh Luke

After the Affordable Care Act passed introducing Value Based Care, Dr. Luke left the C-Suite and founded the not-for-profit National Readmission Prevention Collaborative which continues to provide online educational resources for healthcare executives. Dr. Josh Luke ascended to become the foremost hospital and post-acute authority on the readmission penalty and twice served as executive host of the World Congress on Readmissions. After starting his career in healthcare as a nursing home administrator, Dr. Josh Luke ascended to become a hospital CEO by age 32. Before leaving the c-suite, Luke served in four hospital CEO roles as well as serving as the Vice President of Post Acute Services for a non-profit health system. During his more than ten years in C-suite healthcare roles Dr. Luke oversaw acute rehab, behavioral health, home health & hospice and is an advocate for non-medical home care. Dr. Luke was later asked to design post-acute coursework based on his best-selling readmission book and serve as Adjunct Faculty at the University of Southern California’s Sol Price School of Public Policy.
