The Affordable Care Act introduced two Alternative Payment Models – bundled payments and Accountable Care Organizations. It also introduced the first ever Hospital Readmission Penalty, Medicare Spending Per Beneficiary Penalty and other penalty program to speed up the transition to value based care. Specific attention is given to the new, 2019 CMS discharge planning rules that require hospitals to meet certain communication, medical record and transfer requirements.
